To submit additional address information (i.e. mailing address, email, emergency contact), use the Additional Address form, and bring or mail it in to
the Registrar, or Fax to 708-225-5806.
Have you decided to change your curriculum? Fill out a Curriculum/Education Objective Form and bring or mail it in to the Registrar, or Fax to 708-225-5806.
Need transcripts sent somewhere? Complete a Transcript Request Form, and send it along with a payment of $5.00 per mailed and picked-up transcript. If you are faxing, include your credit card number, date of expiration and signature and fax to 708-225-5806.
